Worst: Sending material back to an author, who is convinced that their paper is superb, requesting either substantial re-write or submission to another publication
Amusing: I once sent a paper back to an author requesting some small changes and received a return email reminding me that not only had it already been accepted but the author had just approved and returned the galley proofs! I replied with an apology, saying that my email had been sent in error! Taught me to keep a good filing system.

Strangest request: I once spent nearly two months helping to translate a paper into English. When it was ready for publication the author asked me if I would like to be credited as the author.
