In one of my first books, an outsourcing company that was working on fonts, etc., proudly told me that they had discovered a spelling error - they had changed the word "perioperative" to "preoperative" in the entire manuscript.

If they could list the names of everyone who had reviewed their chapter as authors as they did not want to hurt anyone's feelings.
